WHAT TO EXPECT AT FIRST APPOINTMENT
Before: After scheduling an appointment, you will receive a confirmation text and/or email from our online scheduling calendar (Square) confirming your appointment.
You will receive a second email (from CharmPHR) to access your new Patient Portal. The email will include a link to activate your new Charm Patient Portal, where you can safely communicate and share information with us directly. Your Patient Portal is where you can send and receive messages to and from your provider, fill out New Patient Forms, send us any previous lab results/imaging/medical history, and access any of your important documents at any time.
During: At your first visit, our staff will confirm we have all the necessary forms filled out and signed by you, and will provide you with any remaining forms that need to be completed before seeing the provider.
Your first appointment includes an in-depth discussion of your health history, current concerns, and goals. We take the time to listen, assess, and perform any necessary exams or testing. You and your provider will discuss possible treatment options, what the provider recommends, and what fits best for you. Together, you will develop a general plan including what to expect after todays visit, what to expect from your provider in the next couple of days, next steps, any labs/imaging to have done, referrals, and for when to schedule follow-up appointment (whether it be for a recommended procedure or treatment, lab review, general follow-up, special exam, post-procedure follow-up, etc).
After: Youll receive a written personalized treatment plan from your provider with everything you discussed together, including any recommended medications, supplements, or natural therapies, lifestyle and home recommendations, nutrition and diet recommendations, ordered or recommended lab testing and imaging, referrals, follow-up recommendations, and summary of any recommended or planned procedures and treatments that were discussed with your provider.
